Log CPD

CPD can take many different forms and varies for each individual but your log should contain a clear audit trail of your learning and development activities. You can use the form below to record an activity, identify what you had hoped to achieve and reflect briefly on how it has supported your personal development. You will also need to identify the type of CPD, for example 'My CPD' or 'Professionalism and Ethics'. Don't forget that all members must complete 5 hours of professionalism and ethics CPD each year.

Briefly describe what the learning activity was and what it was about. The activity may have been planned or unplanned.
Reflect on and evaluate the extent to which the activity helped you meet your learning goal. For example: For what reason did you complete this activity? What did you learn? How are you applying what you learned? To what extent has this helped you with your professional development? What further development needs have you identified as a result of completing this activity? What will you do next?
